Providing both Family Medicine and Emergency Medicine Services

Daily clinical services at our Falcon, CO location were discontinued 12/17/10.

Effective 1/1/11 Falcon Family Medicine is only available to those with a private contract.

**If you have a contract for medical services and need to reach Dr. Herbert, please call directly on his private line provided in your contract. For his privacy, we do not publish this number publicly.

At this time we are not accepting any new contracts in order to provide the best services to our current clients.

We are no longer accepting telephone calls from former patients with questions about medical records or patient accounts. Please contact us in writing. A staff member will get back to you as quickly as possible in writing or by phone if you include a telephone number.

To obtain a copy of your medical records please print out the authorization and mail it to:

Please allow 3 weeks for records to arrive at your new physician's office. Records are delivered by courier to local offices.

If it has been 4 weeks since you requested your medical records and your new clinic states they have not received them, please write to us and we will be happy to mail you a copy of the signature page showing who signed for them at your new office.

Please note: There is no charge to send the first copy of your medical records to your new primary care physician.

Per Colorado law, a fee is charged for additional copies or a copy for yourself. A flat fee of $14 per record placed on a CD-ROM + $5.20 shipping. Printed records are per page. An invoice requiring pre-payment will be mailed to you prior to the release of duplicate copies of your records, or copies to yourself.
